When play calling a game how do you substitute a player like a QB for instance?

You don't. I mean I think the game auto-subs when the men reach a certain level of fatigue, but you can't manually do it

I believe on the fly subbing should be in the next version as a real coach who finds a mismatch that he wants to exploit will replace players or pull a player if he's struggling.
