Offense not resetting after some plays

Okay, here's an interesting one. I wasn't sure this was actually affecting things at first, but it appears to def modify the outcome of plays.
After some plays, seemingly at random, the offense won't reset, at least not completely. The offense will be sort of "setup" in an amorphous blob formation, ie no formation, and just run the play seemingly with each offensive player just starting from where they ended the previous play instead of lining up at the specific spot on the field per the called formation.
