If you start a save in 1995 say, will the league expand when appropriate? Or are you then stuck with the number of teams from the starting year?

You can expand the league (up to 32 teams) at any time you want (as long as it is on the one day in the game that allows expansion - that is noted in the key dates screen). So you can expand at the proper time, earlier/later or not at all.
